Are you an active employee? To make access simple & secure, please connect to the Equifax network first at an office location or through VPN to take advantage of single sign-on authentication. To get a copy of your:

• W-2 or 1095-C – Login to the My Tax Form Management. Check the W-2 OR 1095-C box for the tax form needed and then click Download or View to see your form. *Note – if you have not consented to online delivery through the Tax Form Management portal yet, your forms were mailed to your primary home address on file with Equifax in Workday. You can still visit the Tax Form Management Portal (while on the Equifax network) to consent and view & download your tax forms today.

Are you a former employee or an employee without access to the Equifax network? No problem. Please send an email to AskHR@equifax.com with a request for your W-2 or 1095-C along with the tax year needed. We'll be happy to get these documents to you.
